{"id":9976,"date":"2026-09-19T11:45:30","date_gmt":"2026-09-19T09:45:30","guid":{"rendered":"https:\/\/soleyes.es\/?p=9976"},"modified":"2026-09-19T11:52:16","modified_gmt":"2026-09-19T09:52:16","slug":"presbyopia-eye-drops-reading-without-glasses-again","status":"publish","type":"post","link":"https:\/\/soleyes.es\/en\/eye-doctors-advice\/presbyopia-eye-drops-reading-without-glasses-again\/","title":{"rendered":"Presbyopia Eye Drops: Reading Without Glasses Again?"},"content":{"rendered":"<h2>Presbyopia Eye Drops: Could We Read Without Glasses Again?<\/h2>\n<p>From around the age of 40\u201345, many people begin making an almost unconscious movement: holding their phone farther away in order to read it.<\/p>\n<p>This is presbyopia.<\/p>\n<p>For decades, the available solutions have been mainly optical or surgical: reading glasses, progressive lenses, contact lenses or, in selected patients, different surgical procedures.<\/p>\n<p>In recent years, however, a completely different alternative has emerged: <strong>eye drops capable of temporarily improving near vision<\/strong>.<\/p>\n<p>The obvious question is whether they really work.<\/p>\n<p>The answer is <strong>yes, but with important qualifications<\/strong>.<\/p>\n<h2>Eye Drops for Presbyopia: Could We Read Again Without Glasses?<\/h2>\n<h2>Why Does Presbyopia Develop?<\/h2>\n<p>When we are young, the crystalline lens can change its shape to focus on objects at different distances. This mechanism is known as accommodation.<\/p>\n<p>With age, the system progressively loses this ability and focusing at near becomes increasingly difficult.<\/p>\n<p>We initially need more light to read, then begin holding text farther away and eventually require optical correction for near vision.<\/p>\n<h2>How Can an Eye Drop Improve Near Vision?<\/h2>\n<p>This is the most interesting part.<\/p>\n<p>Currently available eye drops <strong>do not restore the youthful accommodative ability of the crystalline lens<\/strong>.<\/p>\n<p>Their principal mechanism is different.<\/p>\n<p>Certain drugs produce a controlled reduction in pupil size. A smaller pupil creates an optical effect similar to reducing the aperture of a camera: it increases <strong>depth of focus<\/strong>.<\/p>\n<p>Near vision can therefore improve while maintaining functional distance vision.<\/p>\n<p>This is known as the <strong>pinhole effect<\/strong>.<\/p>\n<p>The patient may read better, but the crystalline lens has not become younger.<\/p>\n<h2>Pilocarpine: The First Generation<\/h2>\n<p>Pilocarpine was the first drug approved by the FDA specifically for presbyopia.<\/p>\n<p>Its efficacy is now supported by a substantial body of clinical evidence.<\/p>\n<p>A meta-analysis published in <em>American Journal of Ophthalmology<\/em> included four randomised trials involving 1,531 participants.<\/p>\n<ul>\n<li><strong>33%<\/strong> of pilocarpine-treated participants gained at least three lines of distance-corrected near visual acuity.<\/li>\n<li><strong>12%<\/strong> of patients receiving vehicle achieved the same result.<\/li>\n<li>Improvement can be detected within the first hour.<\/li>\n<li>The effect may persist for several hours.<\/li>\n<\/ul>\n<p>This demonstrates that the effect is not merely subjective.<\/p>\n<h2>The Limitation: Miosis Also Has Consequences<\/h2>\n<p>A smaller pupil increases depth of focus but also allows less light to enter the eye.<\/p>\n<p>Some patients may therefore notice reduced visual quality in dim illumination.<\/p>\n<h3>Reduced Vision in Low-Light Conditions<\/h3>\n<p>A smaller pupil may compromise visual quality in poorly lit environments.<\/p>\n<h3>Headache<\/h3>\n<p>Headache is one of the most commonly reported adverse effects.<\/p>\n<h3>Blurred Vision<\/h3>\n<p>Temporary episodes of blurred vision have been reported in some patients.<\/p>\n<h3>Ocular Pain<\/h3>\n<p>Mild and transient ocular discomfort may occur.<\/p>\n<p>There is another issue ophthalmologists must consider carefully.<\/p>\n<p>Cases of vitreomacular traction and retinal detachment have been reported in temporal association with pilocarpine used for presbyopia.<\/p>\n<p>This does not mean they are common, but it supports appropriate patient selection and particular caution in people with retinal risk factors.<\/p>\n<h2>Aceclidine: A New Generation<\/h2>\n<p>In July 2025, the FDA approved an ophthalmic solution containing <strong>aceclidine<\/strong> for treating presbyopia in adults.<\/p>\n<p>Its particular interest lies in producing significant miosis with comparatively less effect on the ciliary muscle.<\/p>\n<p>The available evidence includes:<\/p>\n<ul>\n<li>466 patients enrolled in pivotal efficacy studies.<\/li>\n<li>361 participants in a 26-week safety study.<\/li>\n<li>883 participants included in a 2026 meta-analysis.<\/li>\n<\/ul>\n<p>The pooled data demonstrate a significantly greater likelihood of improving near vision compared with placebo.<\/p>\n<p>However, larger studies and long-term follow-up remain necessary.<\/p>\n<h2>Does This Mean the End of Reading Glasses?<\/h2>\n<p>No.<\/p>\n<p>It is more accurate to regard these treatments as <strong>a new option<\/strong> rather than a universal replacement for spectacles.<\/p>\n<p>Their effect is temporary.<\/p>\n<p>Response depends on several individual factors:<\/p>\n<ul>\n<li>Age.<\/li>\n<li>Refractive error.<\/li>\n<li>Pupil size.<\/li>\n<li>Visual requirements.<\/li>\n<li>Individual ocular characteristics.<\/li>\n<\/ul>\n<p>Some patients may find them useful for specific situations, while others will continue to prefer glasses.<\/p>\n<h2>Who Should Use These Drops?<\/h2>\n<p>These medications should not simply be viewed as drops to purchase and try without professional assessment.<\/p>\n<p>Before using a drug that modifies pupil size, it is reasonable to understand the patient\u2019s ocular status.<\/p>\n<p>The peripheral retina deserves particular attention in individuals with:<\/p>\n<ul>\n<li>High myopia.<\/li>\n<li>Predisposing retinal lesions.<\/li>\n<li>A history of retinal disease.<\/li>\n<\/ul>\n<p>Night-driving requirements and other situations demanding high-quality vision should also be taken into account.<\/p>\n<p>Personalised medicine remains essential.<\/p>\n<h2>Are These Drops the Future of Presbyopia Treatment?<\/h2>\n<p>We are probably witnessing <strong>the beginning of a new therapeutic category<\/strong>.<\/p>\n<p>For decades, presbyopia management focused on placing a lens in front of or inside the eye, or surgically altering the optical system.<\/p>\n<p>Today, we can temporarily modify the eye\u2019s own optics pharmacologically.<\/p>\n<p>Research continues into treatments that are more selective, longer lasting and associated with fewer adverse effects.<\/p>\n<p>However, perspective is important.<\/p>\n<p>Current drops <strong>temporarily improve near vision;
